Each undersigned represents and warrants that the information provided is true and complete, and that AFIC may consider this statement as continuing to be true and correct until a written notice of a change is given to AFIC by the undersigned. AFIC is authorized to make all inquiries it deems necessary, including the obtaining of a credit report, to verify the accuracy of the statements made herein, and to determine your creditworthiness, now and on an ongoing basis. You understand and agree that from time to time AFIC may receive information about you from third parties including statements, confirmation or electronic access AFIC may share the information it obtains through these inquiries and any credit report with other AFIC affiliates to the extent necessary, or advisable. By signing below I/we acknowledge the above representations and agree to apply for individual extension of credit, joint extension of credit, or as guarantors for the indebtedness.
